Advanced care planning is a way to help you think about, talk about and share your thoughts and wishes about future health care decisions. Advanced care planning can help you determine who will communicate for you if you ever become unable to communicate for yourself.

We will walk you through the process and help you create a plan.

Initial consultation 90 min for $150 in clinic, virtual or in-home + mileage  (call (204) 361-5683 to book in home).

In-home support fee: $55 per hour + mileage

Mileage cost is 50 cents per kilometer

Cancellation Fee — Prior to 48hrs, no fee; within 48hrs, 50% of the fee
